If you are on this page, your healthcare provider has referred you for specialized nutritional care. I am Courtney Berry, MS, RDN, LDN, a Registered Dietitian and the founder of Whole Berry Health. My goal is to work collaboratively with you and your medical team to create a personalized, evidence-based nutrition plan that fits your diagnosis, your lifestyle, and your unique goals.
How to Get Started Please use the calendar below to schedule your 60-minute Initial Consultation. To ensure a smooth process, here is what you need to know:
Secure Your Spot: You will be asked to enter a credit card to reserve your appointment. Your card will not be charged today. It is simply held on file to adhere to our practice's cancellation policy and to cover any copays or deductibles after your insurance processes the claim.
Check Your Email: Immediately after booking, you will receive a welcome email containing a link to your secure client portal.
Complete Your Paperwork: Inside the portal, you will find your clinical intake questionnaire and other required forms. You must complete these forms at least 48 hours prior to our session.
Our Cancellation Policy Because we reserve a full 90-minute block on our calendar to review your medical history and conduct your 60-minute assessment, we require a minimum of 48 hours' notice for any cancellations or reschedules.

Medical Nutrition Therapy is an evidence-based medical approach to treating and managing certain health conditions through an individually tailored nutrition plan. This specialized counseling is provided by a Registered Dietitian and is often covered by health insurance when referred by a physician.

Your card will not be charged today. We require a card on file to securely hold your appointment in accordance with our 48-hour cancellation policy, as insurance companies cannot be billed for missed appointments. It is also used to conveniently cover any patient responsibility, such as copays, coinsurance, or unmet deductibles, after your insurance processes the claim.

Because we reserve a full 90-minute block on our calendar to review your provider's referral notes and conduct your comprehensive 60-minute assessment, we require a minimum of 48 hours' notice for any cancellations or schedule changes. Appointments canceled with less than 48 hours' notice or no-shows will result in a late cancellation fee charged directly to the card on file.

We submit claims on your behalf as a courtesy, but it is ultimately your responsibility to understand your health coverage.

If your insurance provider denies the claim, determines the service is not medically necessary, or if your plan does not cover our specific billing codes (CPT 97802 and 97803), you will be financially responsible for the full out-of-pocket rate for the consultation (up to $150).
